Neurology & Brain Health Consultation
A detailed evaluation with Dr. Cardiel, appropriate for migraine or headache disorders, memory and cognitive symptoms, proactive brain health, and other neurological concerns that need a broader assessment.
- Discussion of your principal concern, relevant health history, and clinical goals.
- Neurological assessment and review of pertinent records and prior testing.
- Individualized recommendations for investigation, treatment, prevention, and follow-up.
Starting at $1,000. If your history is unusually complicated, you have several major concerns, or you have extensive prior records, contact the practice before booking so sufficient time can be reserved.
Focused New-Patient Consultations
When a Focused Visit Is Appropriate
The three services below all appear as Focused Consultation in Elation. Your specific area of care is identified in the intake forms sent after booking.
Focused Neurology
$625
For generally healthy adults with one defined neurological concern.
A targeted evaluation for an isolated symptom, such as muscle twitching or an uncomplicated intermittent tremor, without the need for extensive records or a broad neurological workup.
Not intended for progressive symptoms, several neurological concerns, or complex histories.
Menopause, Hormones & Brain Health
$525
Targeted neurological guidance related to hormonal changes.
May address migraine changes, brain fog, concentration, sleep, and other concerns during perimenopause, menopause, planned ovarian surgery, or fertility-related hormone treatment.
Progressive cognitive changes or more complicated neurological symptoms may need a comprehensive visit.
GLP-1 & Metabolic Health
$425
For adults considering or already receiving GLP-1 treatment.
Personalized guidance on treatment suitability, medication options, metabolic health, nutrition, and muscle preservation.
Prescription medications and other services are billed separately. A prescription is not guaranteed.

Individualized Scheduling
More Complex Neurological Concerns
If you have multiple significant concerns, a complicated neurological or medical history, or extensive prior testing and records, please contact the practice first. Your visit can be scheduled individually with time appropriate for your needs. You do not need to determine the appointment length yourself.

In Elation, choose Follow-Up Visit for in-person care or Telehealth Follow-Up for an eligible virtual visit.
Established patients may book eligible telehealth follow-up visits online when physically located in New York, New Jersey, Connecticut, or Florida at the time of the appointment. New-patient telehealth consultations may also be available, but must be arranged directly through the office by phone before scheduling. Florida care is telehealth-only. Procedures & Specialized Treatments
Book a listed procedure only if you are an established patient previously treated for that procedure by Dr. Cardiel.
For Botox, nerve blocks, trigger point injections, infusions when available, or procedures not listed in the calendar, call first. Some appointments are arranged directly and do not appear online.

Before Your Visit
Fees, Insurance & Payment
Cardiel Precision Brain Health is a private, direct-pay, out-of-network medical practice. Payment is due at the time of service. Eligible patients with commercial out-of-network benefits may request a superbill, though reimbursement is not guaranteed.
Consultation fees cover physician professional services. Medications, procedures, laboratory tests, imaging, and other additional services are separate unless specifically stated otherwise. Your appointment type and applicable fee will be confirmed in advance.
