Planning a Visit
Appointments & Access
Understand which visits can be booked online, which are arranged individually, and where care is available.
Are you accepting new patients?
Yes. Cardiel Precision Brain Health welcomes new patients for migraine and headache disorders, memory and cognitive concerns, brain longevity, selected general neurology concerns, menopause-related neurological care, GLP-1 management, and selected post-concussion concerns.
Most patients may begin by booking an in-person consultation online. If you have several substantial concerns, a complicated medical or neurological history, or extensive records to review, please contact the practice for individualized scheduling.
Can I book an appointment online?
Yes. Our online calendar offers two options for new patients: Neurology & Brain Health Consultation and Focused Consultation. Both are in-person appointments in Midtown Manhattan. Eligible follow-up visits and certain established-patient procedures may also be booked online.
For unusually complex cases, multiple substantial concerns, or extensive prior records, please contact the practice so your consultation can be scheduled individually. Additional appointment times may also be available by phone.
What is the difference between a Neurology & Brain Health Consultation and a Focused Consultation?
A Neurology & Brain Health Consultation, starting at $1,000, is the standard online-bookable new-patient visit for a primary neurological or brain-health concern requiring a broader evaluation, such as migraine, memory concerns, or healthy brain aging. It is intended for patients whose needs do not require unusually extensive advance review or multiple complex evaluations.
A Focused Consultation addresses one well-defined concern within a narrower clinical scope. The focused services are Focused Neurology ($625), Menopause, Hormones & Brain Health ($525), and GLP-1 & Metabolic Health ($425).
Patients with especially complex histories, multiple major concerns, or extensive investigations should contact the practice for an individually arranged consultation. You will not need to select an appointment duration yourself. Review consultation fees.
How do I select menopause, GLP-1, or focused neurology in Elation?
Select Focused Consultation in the Elation online calendar. This is the shared booking category for Focused Neurology, Menopause, Hormones & Brain Health, and GLP-1 & Metabolic Health.
After booking, please complete the patient intake forms and indicate which focused service you are requesting. The practice will review that information and confirm the applicable fee before your visit. All new-patient focused consultations are in person.
What if I have multiple concerns or am unsure which visit to choose?
If you have one well-defined concern, a Focused Consultation may be suitable. If you need a broader evaluation of your primary neurological or brain-health concern, you may book a Neurology & Brain Health Consultation online.
For several substantial or unrelated problems, a complicated medical or neurological history, or extensive previous testing or records, please contact the practice before booking so Dr. Cardiel can reserve an appropriately tailored appointment. If you are simply unsure which category to choose, we are happy to assist.
I previously saw Dr. Cardiel at another institution. Can I book a follow-up?
Patients previously treated by Dr. Cardiel, including at NYU Langone, may be eligible to book a follow-up visit for an established concern, even if it is their first appointment at Cardiel Precision Brain Health.
Eligible follow-ups may be booked in person or by telehealth. A substantially new concern, major change in symptoms, or a complicated reevaluation may require a different visit type or individual scheduling. The practice is independent and is not affiliated with NYU Langone Health.
Where are in-person and telehealth visits available?
All new-patient consultations are in person at 6 East 45th Street, 4th Floor, New York, NY 10017, near Grand Central Terminal. This includes both Neurology & Brain Health and Focused Consultations.
Telehealth booking is available only for eligible follow-up visits when the established patient is physically located in New York, New Jersey, Connecticut, or Florida at the time of the visit. Dr. Cardiel is registered in Florida as an Out-of-State Telehealth Provider; Florida care is telehealth only and, under the practice's current scheduling policy, limited to eligible follow-ups.
Can I book an initial consultation by telehealth?
Not at this time. New-patient neurological, menopause-related, and GLP-1 consultations are scheduled in person. Telehealth is reserved for eligible follow-up appointments with established patients located in NY, NJ, CT, or Florida. Please contact the office if you are unsure which appointment type applies to you.
What are your office hours?
Appointments are offered Monday through Thursday, 9:00 a.m. to 5:00 p.m. Friday appointments may be arranged for established patients. Saturday mornings are reserved for eligible practice members by prior arrangement. The office is closed Sundays.
Friday and Saturday access is not part of standard online new-patient scheduling. All visits are by appointment; availability may vary.
How quickly can I be seen? Are physician referrals welcome?
The practice maintains a limited-volume model. Reserved priority access, often within 72 hours, may be available for established patients and patients referred through select physician relationships when clinically appropriate and subject to availability. This is not a guaranteed appointment window.
Selected adults with recent concussion-related neurological symptoms may also be considered for timely evaluation. Physicians are welcome to discuss a patient referral or a future referral relationship.

Clinical Neurology
Neurological Care & Procedures
Personalized neurological assessment and selected office-based treatment, guided by clinical need.
Which neurological concerns does Dr. Cardiel evaluate?
Core areas include migraine and other headache disorders, brain fog, memory and cognitive concerns, and neurological symptoms that overlap with sleep, hormonal transitions, or metabolic health. Other selected neurological concerns may be appropriate for a Focused Consultation or a Neurology & Brain Health Consultation.
More complicated medical histories, progressive symptoms, or multiple substantial concerns may require individualized scheduling. The practice is not an emergency or high-volume general neurology clinic.
Do you evaluate patients after a recent concussion or head injury?
Yes. Selected adults experiencing headache, dizziness, light or visual sensitivity, difficulty concentrating, imbalance, or related symptoms after a recent head injury may request neurological evaluation. Dr. Cardiel can assess the symptoms, clarify possible contributing factors, and coordinate appropriate testing, rehabilitation, or follow-up when indicated.
New severe or worsening headache, repeated vomiting, seizures, increasing confusion, weakness, or other emergency symptoms require immediate emergency evaluation—not an outpatient booking.
Which headache procedures are performed in the office?
When medically appropriate, office-based care may include Botox® for chronic migraine, occipital and selected trigeminal peripheral nerve blocks, sphenopalatine ganglion (SPG) blocks, and trigger-point injections. Treatments are selected following clinical assessment, not as an automatic package.
Established patients previously treated with relevant injections by Dr. Cardiel may select the designated Procedure appointment in Elation when available. For a new procedure, infusion, or an uncertain appointment type, please call the office. Procedure charges may be separate. Review procedures and assessments.
Can I schedule an IV treatment or acute migraine injection online?
Please contact the practice in advance to ask about the current availability of office-based acute migraine options, including injectable or infusion treatments. Availability, screening, monitoring, treatment selection, and fees must be confirmed individually.
Specialized treatments are not walk-in or emergency services. Do not reserve an unrelated online visit type for a procedure.
Will I need MRI, biomarkers, genetic testing, or other advanced evaluations?
Not automatically. Depending on the clinical question, Dr. Cardiel may recommend or coordinate laboratory studies, neurological imaging, digital cognitive assessment, sleep evaluation, selected neurological biomarkers, or review of wearable and metabolic information. Testing is used when the result is likely to clarify diagnosis or guide management.
Outside facilities may charge separately, and insurance coverage varies.
Can Dr. Cardiel prescribe or adjust medications?
Yes, when medically appropriate. A plan may include prescription treatment, medication review, nonsurgical procedures, rehabilitation recommendations, lifestyle interventions, or coordination with other specialists. No particular prescription or procedure is guaranteed as a result of booking a consultation.
Will your office automatically have my previous medical records?
No. With your authorization, connected electronic health systems may make some prior records available, but access varies and should not be assumed. You may be asked to obtain relevant notes, laboratory results, imaging reports, or prior evaluations.
The office will provide instructions for securely sharing sensitive records. Please do not email detailed medical records through ordinary email.
An Integrated Medical Perspective
Menopause, Hormones & GLP-1 Care
Dedicated consultations for selected neurological consequences of hormonal transitions and for physician-guided metabolic care.
Can I seek care before, during, or after menopause?
Yes. A neurological consultation may be useful while approaching perimenopause, during menopause, or years afterward—particularly when migraine, sleep, concentration, attention, or perceived memory changes are a concern. Care can also address neurological planning before ovarian surgery, medically induced menopause, or fertility-related hormonal treatment.
When appropriate, Dr. Cardiel coordinates recommendations with gynecologists, reproductive endocrinologists, surgeons, and other treating clinicians. Explore Menopause, Hormones & Brain Health.
Does migraine with aura automatically rule out menopausal hormone therapy?
No. Migraine with aura is not by itself an automatic contraindication to menopausal hormone therapy, but the decision requires attention to vascular risk, age, medical history, migraine characteristics, and the proposed type, route, and dose of therapy. Menopausal hormone therapy and combined estrogen-containing contraceptives are not interchangeable.
Hormone treatment is not routinely prescribed solely to prevent migraine or dementia. Read Dr. Cardiel’s article on migraine during perimenopause and menopause.
Can Dr. Cardiel discuss or prescribe hormone therapy?
When clinically appropriate, Dr. Cardiel may review the benefits, limitations, and risks of hormonal and nonhormonal options and may prescribe treatment or coordinate with your other clinicians. Recommendations are individualized; a consultation does not guarantee a prescription.
A $525 Focused Menopause, Hormones & Brain Health Consultation may be appropriate for a clearly defined concern. For broader neurological assessment, patients may choose a Neurology & Brain Health Consultation. Progressive cognitive changes, complicated neurological histories, or extensive prior records may warrant a consultation individually arranged through the practice.
I already use a GLP-1 medication. Can I still book a consultation?
Yes. The $425 Focused GLP-1 & Metabolic Health Consultation may help selected adults who are considering GLP-1 therapy or already using medication prescribed elsewhere. The visit may address treatment suitability, dose and response, tolerability, nutrition and protein intake, muscle and strength preservation, metabolic health, and long-term maintenance.
When medically appropriate, FDA-approved prescription options and ongoing management can be discussed; prescribing is not guaranteed. Explore GLP-1 & Neurometabolic Care.
Are CGM, DEXA, body-composition testing, or a long program required?
No. Monitoring is selected according to the clinical question. Some patients benefit from assessment of nutrition, metabolic factors, muscle mass, sleep, or physical function, while others need no advanced testing. Tests, equipment, medications, and outside services are not automatically included in the consultation fee.
Longitudinal follow-up may be recommended when useful, but no one-size-fits-all testing package or commitment is required simply to begin care.
Proactive Planning & Diagnostic Clarity
Brain Longevity & Cognitive Health
The distinction between optimizing long-term brain health and investigating concerning symptoms matters.
Do I need to have memory problems to request Brain Longevity care?
No. Brain Longevity & Healthy Aging is intended in part for adults who are functioning independently and want a proactive assessment of factors that may influence cognitive and functional health over time, including sleep, vascular risk, physical activity, metabolism, medication effects, and hormonal transitions.
How is Brain Longevity care different from a memory evaluation?
Brain Longevity begins with proactive risk assessment, a baseline when useful, and a practical long-term plan. Memory & Cognitive Evaluation begins with diagnostic assessment of new or progressive memory loss, language problems, confusion, changing daily abilities, or other concerning symptoms—even when a patient still functions independently.
When symptoms are changing or progressive, diagnostic clarification comes first. Explore Memory & Cognitive Evaluation.
Will everyone undergo cognitive testing, MRI, genetic tests, or Alzheimer’s biomarkers?
No. There is no standard battery of tests required for everyone. Digital cognitive assessment, imaging, laboratory studies, biomarkers, or other evaluations may be considered if they answer a useful clinical question and the results are likely to change the plan.
Test results are interpreted in clinical context; no single score or scan establishes overall brain health.
Can hormonal health or GLP-1 treatment be considered within brain-longevity care?
Yes, when relevant to neurological function and long-term health. Menopausal symptoms, hormone therapy decisions, metabolic risk, and strength preservation during GLP-1 treatment may be discussed as part of an integrated assessment. Dedicated hormone or metabolic treatment may require its own appropriate evaluation, and no medication is automatically included or prescribed.
Explore menopause-related neurological care and GLP-1 & Neurometabolic Care.
Do I have to enroll in a six-month plan or membership?
No. Care can begin with a Focused Consultation or a Neurology & Brain Health Consultation. Follow-up and more structured longitudinal support are offered only when clinically appropriate and desired. Your plan may expand, simplify, or change over time.
Does this replace my primary-care physician?
No. This is specialist neurological and brain-health care. Routine primary care, immunizations, preventive screening, and management outside the practice’s clinical scope remain with the appropriate clinicians.
Financial Information
Insurance, Medicare & Payment
Direct-pay professional care, transparent fees, and careful distinctions between physician charges and outside services.
Do you accept or bill commercial insurance?
Cardiel Precision Brain Health is a private direct-pay, out-of-network practice. It does not bill commercial insurance directly. Payment for Dr. Cardiel’s professional services is made to the practice at the time of service.
For eligible commercial services, patients may request a detailed superbill for possible out-of-network reimbursement. Whether and how much an insurer reimburses depends on the individual plan and is not guaranteed. Fees & Insurance details.
What are the consultation and follow-up fees?
Current published new-patient consultation fees are:
- Neurology & Brain Health Consultation: starting at $1,000
- Focused Neurology: $625
- Focused Menopause, Hormones & Brain Health: $525
- Focused GLP-1 & Metabolic Health: $425
In Elation, all three focused services share the Focused Consultation booking category; the intake forms identify your selected service. Follow-up visits are typically approximately $500, depending on the visit's scope; some focused follow-ups may have different fees. Unusually complex consultations may require individual scheduling, with fees discussed in advance.
Can patients with Original Medicare or Medicare Advantage see Dr. Cardiel?
Yes, when applicable rules are met. Dr. Cardiel has formally opted out of Medicare. Patients receiving professional services subject to Medicare private-contracting rules must sign an appropriate private contract and pay those fees directly. Neither the practice nor the patient submits claims for those contracted services to Original Medicare or Medicare Advantage.
Please contact the practice before scheduling if you have Medicare, Medicare Advantage, or other government coverage so the relevant payment and documentation rules can be reviewed. Read the Medicare policy.
Will my insurance or Medicare cover tests or medications ordered by Dr. Cardiel?
Possibly. Laboratories, imaging facilities, pharmacies, equipment suppliers, and other clinicians bill separately. Coverage depends on medical necessity, ordering requirements, network status, prior authorization, and the rules of your commercial insurance or Medicare plan. Medicare Part D or Medicare Advantage drug coverage is governed by the applicable drug plan.
Patients should verify facility participation and benefits directly with their insurer before receiving outside services.
Are procedures, medications, imaging, and advanced testing included in the consultation price?
Not unless specifically stated. Published consultation fees generally cover physician professional services. Procedures, injections, infusion medications or supplies when applicable, specialized cognitive testing, laboratory services, imaging, devices, and services by other professionals may carry separate charges.
Any additional charges from the practice will be discussed before treatment. Outside entities set and bill their own charges.
Can I receive an estimate of my expected charges?
Yes. Appointment type and applicable professional fees are confirmed before scheduling. Patients who are uninsured or elect not to use insurance can request a written Good Faith Estimate when eligible under federal rules; it is also provided in other circumstances required by law.
Choosing Your Neurological Care
Practice Fit & Care Model
A limited-volume, physician-led practice for adults who value diagnostic depth, continuity, and thoughtful coordination.
Who will conduct my consultation?
Every consultation is personally conducted by Dr. Myrna Cardiel. She is board-certified in Neurology, Clinical Neurophysiology, Lifestyle Medicine, and Obesity Medicine, and is a Menopause Society Certified Practitioner (MSCP). Her practice combines neurological expertise with a broader medical perspective when clinically relevant.
Do you see general neurology patients?
Yes, selectively. For otherwise generally healthy adults with one uncomplicated neurological question, such as an isolated twitch or straightforward intermittent tremor, a $625 Focused Neurology Consultation may be appropriate. A Neurology & Brain Health Consultation is available online for broader evaluation of a primary neurological concern.
Complex or progressive presentations, multiple major concerns, or extensive records may require scheduling individually through the practice or referral to a more appropriate specialist. The practice remains centered on migraine, headache, cognitive health, and healthy brain aging, with selected hormonal, metabolic, and post-concussion care.
Can my physician refer me or discuss collaboration with Dr. Cardiel?
Yes. The practice welcomes appropriate referrals from treating physicians and offers a separate way for clinicians to introduce themselves and discuss potential future referral relationships. With patient authorization, Dr. Cardiel can coordinate findings and recommendations with the referring team.
Referral details and records should be exchanged through an approved secure method rather than ordinary email. For Referring Physicians.
Why might I choose direct out-of-network care?
The limited-volume model preserves time for detailed history-taking, clinical reasoning, education, thoughtful treatment planning, and direct involvement by Dr. Cardiel. Patients may begin with a Focused Consultation or a Neurology & Brain Health Consultation; more complex needs can be arranged individually. Follow-up is based on clinical need rather than a predetermined package.
Does the practice provide emergency, urgent-care, or 24-hour on-call services?
No. The practice is not an emergency department, urgent-care center, walk-in infusion clinic, or 24-hour on-call service. For symptoms that may indicate stroke, sudden severe neurological illness, or a dangerous complication after head injury, call 911 or seek immediate emergency evaluation.
Priority scheduling and individually arranged treatment visits do not replace emergency services.
For Emergencies, Call 911
Cardiel Precision Brain Health is not an emergency or on-call service. Sudden weakness, trouble speaking, severe or abruptly worsening headache, serious symptoms after head injury, or other potentially life-threatening changes require immediate emergency evaluation.
